how about an invisible guy in a hospital? you could have some fun with scalpels and needles in eyeballs, ie blinking and looking round with the needle still poking out of the eyeball.

Exactly, don't make a film just becuase you want gore. Make a film becuase you want to do a zombie film, and then have gore, or do a film becuase you want war, and then have gore.
They're right...Take a look at Audition...That movie has one heck of a buildup to an ultra-gory ending...Such a brilliant movie...But it doesn't base itself off of disturbing imagery, violence and gore...People actually walked out of the theater when it was shown because of the unexpectedness of it...
Actually you should do a realistic look at war in the twenty first century. Action movies give us the watered down version of war. Real war is worse than anything you'll ever see in a horror movie. Go with that if you want the goriest movie ever.
